The following forms are to apply for your full-time undergraduate finance, as a new or continuing continuing tuition fee and living cost student. This includes Tuition Fee Loan, Maintenance Loan, Maintenance Grant and Special Support Grant

UKPS
UK Passport Details Form 2025 to 2026
If you have a UK passport but did not include your passport details in your application, you’ll need to send a UK Passport Details Form. Do not send your passport to us.
If you have a ROI passport (but do not have a UK passport) you will need to send it to us.

Birth/Adoption Certificate Form 2025 to 2026
Form to help prove your identity if you don't have a UK or ROI passport. You will need to send your original birth or adoption certificate with your completed form.
